Mechanism of Action (MOA)

A long-acting interferon designed to act through interferon alfa signaling in polycythemia vera

  • BESREMI® (ropeginterferon alfa-2b) is a type I interferon that binds to the interferon alfa receptor (IFNAR), activating intracellular signaling pathways (including JAK1, TYK2, and STAT proteins) that regulate gene expression. 
  • These effects are associated with inhibition of hematopoietic progenitor cell proliferation and modulation of cytokine activity. 
  • In some patients with polycythemia vera, BESREMI® treatment has been associated with a decrease in the mutated JAK2V617F allele burden. 
  • These actions may contribute to its therapeutic effects.

Long-acting dosing supported by pharmacokinetic profile

  • BESREMI® is a long-acting interferon alfa-2b with a terminal half-life of approximately 6–10 days.
  • Administration is via subcutaneous injection every 2 weeks during titration and maintenance, with dosing individualized based on response and tolerability.
  • After ≥ 1 year of hematological stability on a stable dose of BESREMI®, the dosing interval may be extended to every 4 weeks, as described in the Product Monograph.
